A Case of Biopsy-proven Young-onset Alzheimer¡¯s Disease with Superimposed Parkinsonism

Abstract

Extrapyramidal signs(EPSs) frequently accompany Alzheimer¡¯s disease(AD). However, the pathogenesis or pathologic basis for the EPSs in AD remains unclear. We report a case of presenile onset progressive dementia later associated with
hemiparkinsonism. Brain biopsy showed neurofibrillary tangles and amyloid plaques, compatible with Alzheimer¡¯s disease. There are a few reports about dopamine transporter imaging of AD with EPSs. I-123 IPT SPECT revealed a decreased uptake in both basal ganglia, more severe on the left side and in the caudal putamen, which is consistent with the finding of idiopathic Parkinson¡¯s disease. Alzheimer¡¯s disease and Parkinson¡¯s disease are thought to be coexistent in our patient.
